WORKING PAPERS
If you are a Highland Park High School student and wish to obtain working papers, you may print out the attached A300 form. The directions on how to fill out the forms are as follows:
1. The student and his/her family must complete section A
2. The student must have section B filled out by his/her employer
3. The student must have section C filled out by his/her physician
4. Once sections A,B,C are complete, the student should bring his/her working papers and a copy of his/her Birth Certificate to Ms. Dewhirst in Guidance. She will complete the form and return the completed copy for the student to give to his/her employer.

*If you are a resident of Highland Park but DO NOT attend the Highland Park High School, you must have section E filled out by the Principal of the school you do attend.
